Ordering of cortical and subcortical gain rows not respected

During calculation of a gain matrix for cortical and subcortical regions, the combination is concatenated without respect for the order of the nodes.  This is probably historically a undocumented convention in the demo data, but it should be fixed as it affects particularly all projection based monitors.
